We ask that you complete the paperwork prior to your appointment and bring it with you to your appointment. You will receive a confirmation call and/or text the day before your appointment to confirm it.
Appointment checklist: (please be sure to bring to your appointment)
- If the patient is a minor (under the age of 18) they must be accompanied by a parent or guardian.
- Completed paperwork
- Detailed medication list completed (prescription and over the counter medications)- Insurance card(s) and a picture ID
- Current prescription glasses and sun wear
- Wear your contact lenses to your appointment. Bring your current eye care solutions with you.
